Hi Jars, I'm not sure about the requirements of your kids coz I don't have any yet. Try to back read so you'll know ano ano sinubmit nang mga ibang ka-forum natin na may kids na. But for you, you may submit the following:
1. Document Checklist for a Work Permit
2. Form IMM 1295 Application for Work Permit Made Outside of Canada
3. Form IMM 5645 Family Information
4. Work Permit Information Form
5. Manager’s Check Amounting to Php 6,450.00
6. Your Original Passport
7. Photocopies of the Passports’ Information Pages and the Pages with Stamps
8. Two (2) Photos
9. Proof of Funds (Ex: Bank Certificate mo at nang husband mo, pay stubs ni hubby, T4 nya, etc.)
10. Additional Documents:
- Husband’s Work Permit
- Husband’s Employment Contract
- Husband’s LMO
- Husband’s Passport (Photocopies of the Information Sheets of His Current and Old Passports and the Pages with Stamps)
- Your NBI Clearance (Original)
- Your Birth Certificate (Original and Photocopy)
- Marriage Certificate (Original and Photocopy)
- Contract to Sell of an Avida Condominium Unit (Original)
- Your College Diploma (Original and Photocopy)
- Your Transcript of Records (Original and Photocopy)
- Your Certificates of Employment (Original)
I also submitted kasi photos of me and hubby plus remittance slips kasi we just got married last 2009. Para lang to convince the VO that we are truly married. I am not sure with your case since matagal na ata kayog married plus you have kids na pati.
