What are the requirements for port of entry working visa ?

I have :

• job offer letter
• positive LMIA cert
• police certificate
• resume
• medical
• passport

is there anything else I need ??

I've already updated my EE profile with job and lmia detail

can anyone please check to see if I have everything or something missing / current fee etc ?
