First PR Card Shipped To The Wrong Address

Hi everyone,
I uploaded my photo and address on the IRCC portal to get my first PR card, on May 20th, 2021.
I was supposed to receive my PR card after six weeks, but l haven't received it yet.
I’ve contacted IRCC through email, and they said, “Our records indicate your card was mailed to your address in mid-July.”
Apparently there is something wrong in my address and it shipped to the wrong address, what should I do to receive it?

you need to consider your card lost and request a new one to be reissued.
Follow the instructions in the “Replace a lost, stolen or destroyed PR card” section on the page to apply for a PR card.

Do you know through what application I should request?
The “IMM 5451 Solemn Declaration”application ? or the previous one that should be applied for initial PR card?
