you need to consider your card lost and request a new one to be reissued. Follow the instructions in the “Replace a lost, stolen or destroyed PR card” section on the page to apply for a PR card.

If you add the letterhead prior to your employee signature, you should be fine. Just make sure not to modify any part of document after it's signed.

if it's reasonable timing (3-4 weeks total spread across the year) it should be fine.
