Does your Birth Certificate have your father's or mother's full name as exactly on your passport ?

You should be good! In case you want to ensure more than 100% compliance - just back it up with a Affidavit!
Good luck!

Hi,
My birth certificate has only first names - my first name and also my parents first name. Do you think I need an affidavit for back up?

Thanks

Hi, I have similar issue. My first name on my birth certificate is completely different from what I've always used. I have never used the name on birth certificate. Once I've known my birth certificate that's when I found out, my mother had me registered late and applied for a birth certificate. At the time it wasn't centralized. So she had always used this birth certificate to register me in schools etc, until when it was centralized, it turned out the midwife had registered me earlier than my mother did but my mother had no contact with this person.

To cut the story short, I had them legally changed and now my birth certificate has an annotations on the side saying that my name has been changed etc and including the change of name legal docs. All my IDs, diplomas, PCC and passports are in the name I've always used.

Under the forms which I have to fill other names/alias/given names specifially forms IMM008 & IMM1295 should I tick this box and write the name on my birth certificate although I have never used it? Cos the instructions under IMM5466 says "this could be your birth name etc"

Kindly advise.
