At the end of the sheet it shows

Is that normal ?? Can someone who has already done their's check what their's says ?

Mine says:

On (Date) you consented online to using eMedical to process your health examinations.

Same thing: you have consented blablabla. Contact your physician maybe they need to reprint it. Did you sign the paper that you consent to share the information?

You need to sign a consent form to share your info with CIC.

Do you remember that form?

If not then call/visit your physician and ask for it.

I had signed all the forms they asked to. The place I did my medicals is in shambles as I faced a lot of issues including even getting that final information sheet. Will contact them to get a response
