Hi,

Thanks for the reply. But on the FBI website, it is stated that ORI is mandatory. Did you received your PCC from USA without filling ORI?

Thanks

Ordering Fingerprint Cards & Training Aids

How can I order fingerprint cards for my agency?

In order to request fingerprint cards and training aids, you must have an Originating Agency Identification (ORI) Number. If you do not have an ORI Number you will need to contact the FBI, CJIS Division, CCA File Unit, at (304) 625-4543 to obtain one. Once you have been assigned an ORI Number, complete the 1-178, CJIS Supply Requisition Form, to order fingerprint supplies and/or the 1-178a, CJIS Fingerprint Training Aids Supply Form, to order training materials. Fill out the forms below and submit them electronically with the SUBMIT button. You may also print and fax completed form(s) to the CJIS Division, Fingerprint Supply Center (FSC), at (304) 625-3984. If you have questions, contact the FSC at (304) 625-3983. If you are trying to order cards for an individual, please call our customer service at (304) 625-5590 for help in this matter. The order forms below are to be used by agencies not individuals.

1-178 Supply Requisition Form
1-178a Fingerprint Training Aids Supply form
What is an Originating Agency Identification (ORI) Number?

A nine-character identifier assigned to an agency. Agencies must identify their ORI Number on the requisition forms in order to receive fingerprint supplies and training aids.

Can I receive fingerprint cards without the ORI Number pre-printed on them?

Yes, agencies may request fingerprint cards with or without their ORI Number pre-printed on them. The requisition form has two columns used to identify the quantity of cards they need. One column is for the quantity without the ORI Number and one column is for the quantity to be pre-printed with the ORI Number. However, you must put your ORI Number at the bottom of the form even if you want them left blank.

Is there a cost for the fingerprint cards?

There is no charge for the fingerprint cards and training aids.

How are the items packaged?

FD-249 Criminal Cards

2000 per box

FD-258 Applicant Cards

2000 per box

FD-353 Personal Identification Cards

2000 per box

FD-884 Palm Print Cards

2000 per box

FD-884a Supp. Finger & Palm Print Cards

2000 per box

Live Scan Cards

2000 per box

R-84 Final Disposition Report

4000 per box

I-12 Flash/Cancellation Notice

1000 per box

1-433a Supervision Transfer Notice

1000 per box

Manila Envelopes

500 per box

Blue Envelopes

500 per box

American Bankers Association Envelopes

500 per box

Agencies may order as many cards as they need. Supplies must be ordered using the unit of issue "each." For example, if you need two boxes of the FD-249 Criminal Cards, the quantity order must be 4,000. However, the maximum allowed for training aids is 30.

How long does it take to process my order?

Orders are normally processed and shipped within 2-3 business days. Most orders are shipped using the FedEx Ground. Be sure to include a complete physical address and telephone number when completing the requisition form(s). FedEx Ground will deliver your order within 1-6 days depending on where you are located.

How do I check the status of my order?

Call (304) 625-3983. If you get voice mail, leave your name, telephone number, and ORI Number and someone will return your call.
