Finding a Supervisor for admission

Hi everyone,
I am applying to graduate course in Pharmacy in uni of sasketchwan. they are asking to get a supervisor. i have found a supervisor. My question is: how should i approach him. What should i write in email? Can anyone help me with a proper format please. also what are the chances of approval.

It's the same style as a SOP - why do you want to study there, why you want to study this major, why you picked this professor, what do you want to focus on...etc

I think u can approach him in a semi-formal manner. just write:

Dear Mr. or Ms.

I applied for Pharmacy course at uni....... I have been asked to find a supervisor for my research paper/thesis/dissertation by the admission office. i would like to know if you would agree to be my supervisor. Please find attached my research proposal.
