Filling Personal Particulars Proforma for the Police verification

I am filling the Personal Particulars Proforma (http://www.cgisf.org/pdf/ppertpro.pdf) for the Police Verificaiton and i have confusion at one of the fields :
8(b) Present Residential Address & Tel. No, along with Police Station and residing since

For this Field should i put my USA address or India address itself? I have been residing in USA for more than 3 years now. Since this verification is for INDIA, is it OK to write your present address as : US address?

9. If you have not been resident at the address given at COLUMN 8(b) continuously for the last one year, please furnish other address(es) with duration(s) resided (Please furnish an additional set of P P Forms for each address with Police station.

Also for the point-9, what do they mean by -(Please furnish an additional set of P P Forms for each address with Police station.

Hi Sandeep - did you manage to clarify your queries above? Since I have the same doubts and I can't seem to find any clarity on it.
