Filling out forms - sponsor

A couple of question - just filling out the sponsor evaluation. Under current employment situation it asks to click on A,B or C which best describes your current situation. I'm presently self employed but also have an employer where I work part time evenings - do I fill out both B and C or just C as its my main source of income.

Lastly, on the document checklist requirement it asks for a copy of my passport. I'll be submitting a photocopy of my passport - does this copy need to be certified?

I would click both options and maybe on a separate peice of paper explain the situation (attach the separate paper to the form)

You do not need the photocopy of your passport certified.
