You can use a blank piece of paper. Just put your indentifying information at the top of it (client ID, name, date of birth) and the question # you are continuing. Put it in the application right after the page you are adding to.

Under the Backgroud/declarartion form, we have to fill out the (8) personal history section.

If i was travelling or unemployed, what should i put on the NAME OF COMPANY, EMPLOMYER, SCHOOL ?????

thanksssss

Also, i have to list down all the recent address, And i was living in someone's apartment 3 years ago in another city and i cannot remember the full address and postol code, what should i put or do ??? I do NOT have any contact with that person ...

THANKSSSSSSSSSSSSSSSSSSSSSSS

You leave it blank or put "N/A" if you were not working or studying.

Put as much of the address as you remember and let them know you can't remember the complete address.

So i just have to put down the addresses that i can remember right ????
I hope that i won't be penalised for that..

Also, I'm SINGLE with no kids, do i need to fill out the •Additional Dependants/Declaration [IMM 0008DEP] form ????

Also, what do they meant by FAMILY MEMBERS filling out on the form ?? Does it involve my parents and my brothers ???

THANK YOU SO MUCH FOR YOUR HELP.................

You have to put down ALL addresses. If you can only remember part of the address, put down that part. If it was in Canada, you can use Canada Post's search tool to either find the postal code or other parts of the address
http://www.canadapost.ca/cpotools/apps/fpc/personal/findByCity?execution=e1s1

No. As stated in the document checklist,
"Completed by the principal applicant, if you have more than five (5) family members."

Also, what do they meant by FAMILY MEMBERS filling out on the form ?? Does it involve my parents and my brothers ???

On which form? In general, unless otherwise requested, "Family" refers only to spouse and children.

The one exception - Parents and siblings are specifically requested on "Additional Family Information (IMM 5406)" (assuming your siblings are not your dependents).

Hey Jess, thank you so much for your help..

1) As for the address, i stayed at someone's home or stay at my friends apartment (the friend have left Canada for good) for 1-3 years and i don't remember a SINGLE address or street number or street name. What should i do ????????? I'm afraid that CIC will check it...

2) So i'm NOT NOT NOT going to fill out the - Additional Dependants/Declaration [IMM 0008DEP] form

3) On the GENERIC application form IMM 0008, they ask me to either ADD or REMOVE the dependants, therefore, i remove ALL the dependants COZ i have no spouse or children... RIGHT ??????

THANKS JESSS,,, YOU ARE THE BEST.................

Yes, CIC will check it and your inability to provide the information could be a problem. Sorry, you need to try harder, contact your friends, google yourself online ... I can find the site (and address) of my first childhood home on Google Earth (now bulldozed). I can find many of my old addresses on whitepages.com (or www.whitepages.ca. Go to a library and ask an information specialist to help you. There will be records of your addresses somewhere, and CIC will find them if you don't...

I had to come up with 25 addresses covering 30 years (since I was 18) and I did. The "I can't remember' excuse will only get you so far.

Sounds right, I did not fill out this new form, but it sounds right.

Thanks for your adivce Jess...

I will try to go to my bank tomorrow and ask for all my address in Canada during my 7 years stay here......

I'm just wondering, how will they know if i live in my friend's apartment, even though i put down his full address. He have left Canada 3 years for good ... The apartment was under his contract....

Just curious...

THANKS JESS for your wonderful HELP....

Did you ever receive mail in your name at that address?

I went to my bank today and asked to to print out all my previous address... I think i got it..
My friend told me that CIC will check through the credit bureau FOR THE ADDRESSS....

thanks Jess for your wonderful help... thanksssss..............

That's great - didn't think of the bank, good to know it worked for you!

It can be 2 pages, but I think it's good to keep it to one page if possible - you can say what needs to be said in one page. Your boss does not have to introduce himself, although it might be good to state what his work relationship is to you - just a single sentence stating I am "rpun's immediate supervisor" or "I am the manager of the unit in which rpun works". Remember, he should attach a business card to the letter as well.
