You need to send one set of each document no need to send two copies. For employment and licensing you need to send one form only no need of duplicating it. Just you need to get two sets of forms filled by your school as this is the only form which is different for RN and LPN. After the NNAS assessment.. you still need to send your result to the provincial regulatory body of choice which need to look into your documents to see if may gaps or lacking sa education mo.
