I also received medical request but no forms.
I received the following:

RequestLetter m - 2 page
CIIP Letter - 2 page
medical Instructions A - 2 page

what is CIIP

CIIP stands for Canadian Immigrant Integration Program.

As per the MR, you need to take only the copy of letter/email listing your application number along with other requirements like passport/photos etc. No forms are required.

Read the instructions in the section: "What you should bring:"

you should Read the medical instructions that they sent you by email,
Here is the checklist that i sent:
Copy of Medical request with file number ( They sent by email)
Photocopy of Medical forms filled and stamped( Given by Hospital)
Photocopy of passports which I presented at the time of medical to hospital ( Photocopies of your passport simply )
4 latest photos of each member according to new photo specification ( different than old specification)
All passports ( even expired ones are compulsory )
RPRF draft
sent at PO box address through Blue-dart and don't forget to paste bar code sticker, if you have ( sent by CHC new Delhi when they sent us file number)
