Obtain the latest reference letter form your reporting head. The reference letter should mention your designation, job duties, current salary and the date of joining.

Attach the business Card / Visiting card of signing Authority. Also, make a photocopy of your company ID card and send it along with.

I understand you are getting salaries in Cash but, if you have salary slips, form 16 and previous year's tax return then send them.

Besides all these you can write an explanation letter or a simple cover letter. Tell your consultant to do this job for you. What is getting pay for?

I think a current dated reference letter mentioning your Duration, designation of employment along with Salary is needed. Get it on Letter head along with business card. Collect your tax returns, and other documents like appraisal letters, training letters etc and submit it to CHC. You may also provide link to your social profile where they can find your co employees recommending you.

Assure CHC that your employment is genuine and you can provide them a specific document that they may ask for
