Few questions about application form & experience certificate!

1. Please tell which form do i need to fill while applying for immigration under skilled worker category.Do i need to fill separate form for my spouse and each dependent.
2. I had applied in 2002 and was rejected. Can the same experience certificate copies be attached for the experience upto that time or i need to obtain fresh exp certificatyes again.Do these certificates need to be notorised or can be produced on just original company letter heads.

1- schedule 1 & additional family info should be filled by both the principle applicant & the spouse. other forms to be submitted by the pr applicant only.

2- you should have the ex later updated( recent not more than 2-3 months old)
