Few queries regarding sending passport . Please see

Hello All,

I have submitted application for myself,wife and daughter and got request from them to send passport for all of us three

Below are some clarifications needed.

1. Do I need to send passport in 3 different envelopes with 3 different consent forms

2. Also what do I need to send to VAC and what to NY or LA consulate. It's very confusing on their website

Kindly help.

Hello dear,
U need to submit two consent letters appropriately signed per individual like in your case you need to send 6 letters and signed two letters by each candidate. Along with the letter of cic in which they demand your passport and 3rd thing is the passports. These three must be sent to new york or la which is convenient for you. Along with these three things you must send the photo copy of the moneyorder or cheque or bankdraft of $30.59 which must be signed on the backside with the pencil. Like for 3 passports you need to send 3 money orders or chequed whatever of amount $30.59 each and NOTE ONE THING you should send the original money order or cheque on the adress

The following are the required documents that need to be submitted along with your original passport:

– Letter from CIC requesting you to submit your passport for processing. You will have received this message through your MyCIC account.

- VAC Consent Form: Please note, the consent form must be physically signed.


- VAC Fee: Payment in the amount of $30.59 USD PER APPLICANT. The VAC fee can be paid by bank draft, cashier’s cheque, certified cheque, or money order directed to CSC Consular Services Inc. Please submit a photocopy of this payment along with your application to our office. The original cheque(s) must be sent to the following lockbox with your name written on the back with a pencil:

-Air Way Bill:

If you would like to use Fedex for your return method, the Airway bill can be purchased through the VAC by following the instructions below or purchase a prepaid Airway bill using a USPS OR UPS envelope.

one thing is my daughter is just 11 months , so i could sign her consent form. Right?

Also to whom name do i need to get cheque or bank draft for $30.59.

Hello,

Can anyone please confirm udon cashier Cheque we need to sign on the back or I can write name in the memo field which is for informational purpose only .

Because bank said we cannot write on the back of cashier check
