Okay... I rethought about why I did not fill this up.
This was my thought process - In many countries when a child is born - usually they give the name Baby Smith or Baby Walker and issue the birth certificate in the hospital on the same day - which is in fact the Name at Birth. After this they change their name to let's say - John Smith or Jack Walker which is what they use for all documents which becomes their legal name and the name at birth doesn't appear in any other documents. I know this because my daughter was named as Baby Mother's Surname in the hospital books but I added the correct legal name in her birth certificates and hospital records as well.
Yeah, you should be OK. Your daughter's one and only name should suffice here
