Buy a mailing label, usually a sticker, and right your name and address on it.

There's a specific size of envelope in the checklist, look for it in dollar store sometimes Dollarama don't have that size, I bought mine in Dollar Tree.

If you forget to sign documents, incomplete requirements as per the checklist, incomplete pictures, it may vary.
Check hundred times (hahahaha) before sending. It made me paranoid too but all went well on mine.

Just to fit the normal font or size of the addressee part of the envelope.

The process was very simple and short, taking about an hour, most of which was waiting time. Taking photos, urinalysis, physical exam, and blood test at the doctor's for $180, and chest x-rays for another $50 at a location two blocks away. So she was right saying $230 includes everything.
