Thanks for the suggestion! Well I don't think that's necessary, because the officer who called me told me her name and initials, and when I am resending the forms, I have to address it to her, and she will be taking care of everything. Additionally she said she's gonna put a note for the Singapore office, to explain what happened...

Now that this ordeal happened, I guess I have the opportunity to be even more thorough than when I sent the application in January.

My questions are:

1. Do sponsors need to fill out IMM 5669 and IMM 5406??

The instruction says that the principal applicant's spouse (whether accompanying to Canada or not) should fill out both forms. Well I AM the principal applicant's spouse (husband), which means I do need to submit them both no??

I think most sponsors don't send them...but I just wanna be totally sure!

2. IMM 5406 ===> Well my wife is married to me, so obviously she shouldn't sign section A. Do you guys (applicants) leave the signature and date fields (of that section) blank, or put "N/A"?

fandev I'm very sorry to hear of this problem with CIC-M. I am really disappointed to hear they are not pulling out all the stops to find the misplaced packages. They were obviously separated, someone will open them in a month or two and see your application is already proceeding and throw them out. It is very unprofessional.

Regarding the two forms you specifically ask about.

fandev Form IMM5669 AND IMM5406 are to be filled out by the APPLICANT not the sponsor.

The sponsor is the person who already has status is Canada - Canadian citizen or Permanent Resident.

The principal applicant is the one who wants to have the right to come living permanently in Canada with their spouse.

The Principal Applicant fills these forms.

Here is the CIC link which lists the Sponsor forms and the forms for the person applying to Immigrate (Principal Applicant).

As JudyO has already answered part of the question I can only re assure you that your wife does not have to sign or fill the date in that section if she already mentions you in the 5406 app.

so I thought, since I am technically the principal applicant's spouse, then I'd have to fill them both out too ;). But thanks for clarifying that I (the sponsor) don't have to fill them out!

Just out of curiosity though: under what circumstances would the principal applicant's spouse need to fill them out?

No she does not need to sign or date it because the question is actually saying only if she did not mentioned you as a spouse in the section above.
As long as your name appears in the Section A. (line 2) Spouse or Common-Law partner you are fine.

You know fandv...Few months ago when I was filing these forms I was also stuck here not knowing how to do it. Its most natural!

And about forms that don't apply to me (use of representative, and statutory declaration of common law relationship): in January I did not include them (to save papers and ink :P).

I assume you guys also don't include forms that are not applicable?
 
Didnt use a representative? Don't include the form!

Haven't got a commonlaw partner that is sponsoring the principle applicant also? I assume not as you are applying comminlaw. Don't include it! :)

I still haven't received mine, and am feeling a bit worried and left behind! But I know that the delay probably comes from that missing paper :(

I am shocked to hear what happened to fandv! You are a bright-spirited individual !! :) Maybe things will finish off a lot faster this time :) Always stay positive fandv.

Gary, can you mark us BLUE???? I just got the official letter that we have SA!!!! The official approval date was March 15th. :D :p

Apparently they send the letter via email to my lawyer since he is listed as the mailing address. I got to thinking tonigt that the letter should have arrived by now but it hasn't showed up at my wife's addy. I contacted my lawyer and he said he got it and forwarded to me last week but it never got to me!!!! Anyway he re-forwarded it to me and i have official SA!!

Note to anyone who used a lawyer or who's home address and official address on the ecas do not match that you need to check the official mailing address for the important stuff!!

More SA's and good news needs to come. Messenger, i know yours will be coming soon!! I have you in my thoughts as well as fandv and anyone else still waiting!!

Hi everyone....

1. To those you (principal applicants) who are not from English speaking countries: did you translate your passport's photo page...?! I guess, no right? My wife's passport is written in Indonesian, but it's got English captions as well. For example:

Nama / Name: bla bla bla
Kode Negara / Country Code: bla bla bla
Tempat Lahir / Place of Birth: bla bla bla

Overkill to translate using a sworn translator, I suppose?! (We didn't do it in January)

2. In January, we did provide a certified copy (of the photo page) though. But was that even necessary...? How do you guys do it?

3. In January, we got a notary public to print a certification on my wife's police certificate, certifying that indeed it was original. Do you guys do it too, or may be it's not that necessary because we're sending an original police certificate?

(and of course we're going to send its sworn translation ;))
