When did you send it to Ottawa ?

Either they misplaced it or it was 3 months old at the time of submission.

Indian embassy issued on 17 jan 2013. i have submitted on 4 feb along with application

i thought pcc is good for an year
 
They may have misplaced it. Send an email to them that you had submitted along with application. Send a scanned copy if you have so that they can match it in the file.

Also, order gcms - it should tell why issued request for PCC. Most probably, you'll see a note at the end of your file that PCC is missing and required.

Also, if they respond that your PCC could not be found, request 60 days time for submitting India PCC telling that holiday processing will cause delays in issuance of PCC and processing time can take upto 6-8 weeks (provide them link to High COmmission site, which provides the processing time).

Might be a silly questions but what options do I pick to get GMCS notes?
Access to Information vs. Privacy Act?
Electronic or Paper Files?
I'm a bit confused. I've never done these. Would it be useful to do at this point in time (just received MR/Biodata/RPRF request?)

Its free under privacy act. I believe its $5 under access to information.
Also, if you want to receive it via email, then its electronic files you'll select. Select paper files if you want a hard copy.

You should always apply under Privacy Act if you're looking to find information about yourself.
 
Holy cr*p! Got RPRF and Biodata request today. Can't believe it!

No medical request yet but that seems to be not abnormal from what I'm reading here.
