I have been posting, and mostly reading others posts and threads for a couple of months, and I wanted to say a big THANK YOU!! to every one. You guys have answered so many questions and have been such a huge help.

I wanted to start this thread because my boyfriend has his medical exam on the 13th and we will be sending our application right after that We are going through the Paris visa office.

I also wanted to use this thread for all last minute questions and paranoia.

I think there's only one check list as far as that goes. So yes you have to include everything together.
But don't forget there's country specific checklists as well. I'm not familiar with the Philippines but, you can find it on the CIC website.

The main one you mention is IMM5491E and has a section for the sponsored person. You need to fill this out and it is generic enough to encompass all applications. If is basically a summary of the detailed checklist. Since you (as sponsor) are submiting the entire package it is to ensure you have included everything.

Then there is the detailed checklist in the Country Specific Instructions - Philippines. It is on pages A-1 to A-4 and lists documents specific to the Philippines. The applicant needs to complete this part to ensure all the required forms are with the application they are sending you. Notice that the last checkbox on this checklist is:

A COPY OF THIS DOCUMENT CHECKLIST

And in the instructions they say to send every document mentioned on this checklist. So in the final package you should have two fully completed checklists.

I literally just got back from the post office!
It's been a long few months trying to gather all the necessary paperwork and getting answers to the many questions I've had (a BIG thank you to everybody that helped!), but everything has finally been sent off!

Fingers crossed for a simple, speedy process!

Quick question about the uci/ cliend id. Both me(sponsor) and my partner (applicant) should have one.

1. I am a naturalized citizen, came to canada when I was 10 so that was quite a while ago. Asked my parents but they can't seem to find it. Is it a problem if I do not put it on the form.

2. My partner was in Canada from 2008 to 2010 on a working holiday, and then with a work permit. Should he put in his number from back then or should he leave it blank to get a new one.

Hello.

1. I would suggest do as much as you can to find it, call CIC and see if you can find out, or get a copy of your citizenship certificate/record.

2. Your UCI number stays with you for life. - so in this case, wherever the UCI is located on your partner's papers, reference that number.
