aliases AKA
your no OCA
fbi no FBI
armed forces no MNU (this is NA i think)
misc no MNU

1. AKA are you known by any other names?
2. OCA you don't have one.
3. FBI you don't have one
4. MNU you don't have

Leave them blank.

PMM is right. The FBI will stick a number to your card when they process your application.

One more thing, I'm currently in mumbai for a year. I know pep staying in US under work visa apply to indian embassy in US for PCC. Since I'm in india can I request US consulate in Mumbai to issue me PCC or I've to send request to US FBI office by mail?

US embassies do not handle that. You need to write to the FBI directly.
