FBI Fingerprint Card

Hi,
I need to get FBI Background check done for Canada EE. I have few questions on the same which are as follows -

1. Can I leave the 'ORI' field blank?
2. Can I leave the 'SSN' field blank if I don't have an SSN#? (My wife is on H4 visa and she doesn't have an SSN)
3. Can I leave leave 'Employer and Address' field blank?
4. Can I leave 'Signature of Official Taking Fingerprints'? (I have taken the prints myself)

Any response will be appreciated.

2. yes

4. yes, just make sure the finger prints are acceptable to FBI.. high chances of rejection, if not taken properly..

Thanks for the responses. I have one more question - if I am sending more than one copy of Fingerprint card, do I need to pay $18 for each fingerprint card processing?
