FBI finger print Send back Blank Cards, with information and notary.

Hi to all,
I sent my first finger print card on "Your item was delivered at 11:44 AM on April 23, 2010 in CLARKSBURG, WV 26301."
now they send me 4 blank forms (for 2 people) with cover letter, information steps and notary.
i dont know why? but I have to go back for finger print?
and What about notary? why they ask for notary and what?

Was the name on the Credit Card used different then yours, if yes then you have to notarized the form and send it again with the finger prints.

No, I sent Money orders only. but they send me FD forms for finger prints again.
they send me 4 blank forms for 2 applicant, one cover letter, 1 notary for that disclouse for imformation, one credit card form. last time I paid by money orders and they not return money orders. anyone in same boat that they sent you new blank forms back?

before I downloaded forms form FBI site and get finger print on it, now they send me forms looks same but thick and small paper.

Yep, you're supposed to use the real form.

You'll need to pay the processing fee again, too.

Right , because before I used downloaded form from FBI site, and the local police department in my town they dont have any forms.

Can any one tell me I have to get finger prints on 2 forms (means 2 forms for 1 applicant) because we get total 4 forms for 2 people and single cover letter and single notary letter.

This is their number you might want to give them a call: 304 625 5590. Option 2, then option 4 through 9.

Perhaps, the finger prints were not done properly, or you did not use black pen to fill the forms.

everything was perfect, I guess I wrote my employer address and Purpose is Canada immigration. so, may be this was problem? still dont get reply of my email from customer service.

no cards was the problem .. i also got the request to re-fingerprint and they did sent me the fingerprint card ... and also a sent me a form which said that my fees has been processed and I dont need to pay again .. I just had to send them my fingerprint again along with that instruction form ...

i already got my report

I got blank finger print card, cover letter, but nowhere mentioned that I dont have to pay, even they sent credit card page. and notary for "Authorization for release of information" , what did mean it for notary.
