Read through the entire thread again. What does your ECA report have? Does that match your passport or your educational documents? In my case too my father's name is included on all my documents, so it is like my middle name, in which case it might look like you have two names. So I think it should be you who gets the affidavit. Also, I think you should not say yes to the question: have you changed your name or had a different name? Because that would mean you have gone through the name change process intentionally and have the documents to prove that. This on the other hand is like clarifying the unintentional mistake? That's my take on it.
