i was wondering what to fill out in forms if fathers date of birth is unknown because he deserted us 20 years ago and we never heard back from him ?

I would write in unknown, and add in an explanation saying exactly that.

I found my Father's date of birth listed on my birth certificate. My Father was not in my life either.
