Hi All,

I have a issue regarding 'Family Name'.

On my passport =>
Family Name : <blank>
Given name : XXX Singh

In Generic application Form, under "Personal Details -> Section 1, we have to put a Family Name else we cannot validate the form. Should I put 'Singh' as my family name??

On work permit =>
Family name : Singh
Given Name : XXX

--------------------------------------

Even for my parents its same information on Passport (no Family name)
Father => Family name : Singh, Given Name : XXX
Mother => Family name : Kaur, Given Name : XXX
In Background/Declaration form, What should I put under FAMILY NAME??

Please advise. I am in Canada as Temporary Foreign Worker.

To apply you must have a family name and it should be written exactly according to your passport. You can get your name corrected in your passport according to your work permit and as well as for your parents. It should be very easy, just send your passport to the Indian high commission (assuming you are Indian) in Canada and ask for the amendments. You parents can get their correction done in local passport office in India.

My suggestion would be to put "Singh" as Family name and get the name updated in Passport as well.

With Singh as your given name and XXX as your family name you should be fine..

If you do not have any family name on your passport you can put the full name in surname column. I had the same issue. It is mentioned on CIC website as well. I have copy pasted the answer below from CIC Website as I am not allowed to post the links. Hope this helps.

What do you mean by surname?

Your surname is your family name. It is sometimes called your “last name.” If your name is spelled differently on your official documents, please write your name as shown in your passport. When filling out applications, type your surname as it appears on your passport, travel or identity document or any letters you got from the visa office or case processing centre where you sent your application (even if the name is misspelled). Do not use initials.

If you do not have a surname on your passport, travel or identity document, enter all your given name(s) in the surname field, write “not applicable” or “N/A” in the given name field.

JasleenK is right.
I have the same problem in my passport. Leave the given name column blank in all forms and enter your full name in 'FAMILY NAME' or 'SURNAME' column.

Thanks Jasleen for the information.

Is there any way I can contact you as i need some information related to the forms.

Please write to me : simrat83@gmail.com or inbox me your number .
