is it possible that agency's can make up fake medical exam for applicants???

Upon seeing your profile history, you mentioned that you had a misrepresentation ban. Medical tests are done by the IRCC panel of physicians and not the agency. You need to either ask your agent and/or order CAIPS notes to know the reason of your refusal.

They can make fake medical appointments but IRCC will never see the outcome, as there is no way to pass on the results.
It would only be a method to extract money from you.

But honestly, at that moment I didn't realized that what should I do...that's why I gave them money..know I'm realizing what I have done

CAIPS is short form for Computer Assisted Immigration Processing System. GCMS is short form for Global Case Management System. They both refer to the same type of service.

How did you know you were banned, was it stated in the refusal letter?

Caips/Gcms notes represent same purpose, request yours from caipsgcms.com to find out if actually you got the ban and a revealing insights to why your application was delayed.

yes sir it was mentioned in the refusal letter....and they also mentioned about paragraph 40(1)(a) and A40(2)(a) by IRPA and IRPR
 
Misrepresentation will cost you a 5 year ban from applying to any Canadian visa.

On the other hand, why hire an agent when all the information you need is online?

People just take some time to read and understand stuff. It's funny to see how much easy money these agent make because applicants are too lazy to figure thing out on their own.
