extremely confused schedule A

So i just checked my email, and cic sent me this:






This is in reference to your application for permanent residence in Canada. We are still missing the following information in order to finalize your application:

Schedule A: Please provide a separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature. Please provide a completed form and include all recent information in questions 8 and 12.

The above information/documents must be received in our office within 30 days from the date of this letter.

Please reply directly to this email and put your application number in the title.

This is a final reminder for you to comply with our request. If we do not hear back from you within the prescribed deadline, an officer will review your case and make a decision based on the information available. As a result, your application may be refused.

I had already sent them my schedule A background declaration with my original signature (printed it, signed it, and scanned it, emailed it). Was I doing something wrong? It seems like they were specifically referring to questions 8 - 12 for organizations/gov positions which i wrote "NONE" on all of them, and for work i worked 2 times so i put that. but looking at it now it seems as though it was asking me for my MOST RECENT work first, and I did the opposite of that. could that be why they are asking me to redo it?

Also, I had been receiving disability (before I came to canada in september 2016), which i did not write under work history. does that matter? or do I need to write that or write unemployed?


as for the addresses, it says write where you lived at for the past 10 years or 18th birthday, whichever is closest. I'm not understanding what I did wrong there, or if i did anything wrong. when I was 18 i lived at 1 address, unless it is now asking me to say what address i'm staying at in canada?


I'm not exactly understanding what I did wrong with this form. Could someone also please help me by linking me the schedule A to be SURE that I will be filling out the correct one?

It used to be that the signature had to be 'real' - not scanned and emailed. Don't know if they accept scanned ones now as part of the new application.

Q8 explicitly says "Start with the most recent", and "if you were not working provide information on what you were doing". So if you started with oldest first, and didn't fill in any gaps, then that would be two reasons why it was returned.

Q12, you have to put every address you have ever lived at since you were 18, not just the address you were living at when you were 18.

The thing I don't understand, is that if it has to be real why are they asking me to send it through email? Should I sent one through email and one through mail just to be safe? I don't even know their mailing address they have only ever told me to send everything through email.

And I completely blanked on the address part, I actually had been living with my sister for awhile after I was 18, but then moved back in with my parents. And for me currently in canada (as a visitor) do I also put that in the addresses?

The application has to be filed physically initially. You must have sent it somewhere to start off with, even if other parts are requested electronically now.

If you are living there, you include it. They're not kidding when they say they want a full address history from age 18, most recent first, working back so where you were living at 18 is at the bottom of the list.


and for work history, for me would it go something like this, the first being most recent:

Thank you for your help. I ended up using the older version of it because they accepted it last time, and I made sure everything was filled out correctly this time, and that all dates were accounted for, and now I am sending it

SCHEDULE A--BACKGROUND DECLARATION

6 PERSONAL HISTORY - my wife stared work on 2013 nov on going but when i try to validate it cant passed me to validate.
10 ADDRESS lat 10 years 2009-09 to on going still same problem i am facing.

please help if there any body solve this problem which is acceptable by cic.

any solution will be much appreciated.
