Extension of visitor status while gathering PR information. Help? Confused.

I am currently in Canada and in March will be able to claim common-law for the Family sponsorship application.
Until then I will need to extend my stay as a visitor seeing how my status will be up the exact day that we can apply. I am having a few issues finding information to help me in my current situation because I am 10 weeks pregnant. Therefore I cannot have my medical done for the PR paperwork until after summer.

Questions:
1. On the extension form IMM 5708 in the section details of my visit to canada;
Indicate how long you plan to stay. Would I put my to and from dates from when my status expires I when I would like the granted extension?

2. Background information;
Do you have any physical or mental disorders that would require social and or health services, other than medication?
----> clearly yes, because I am pregnant, but I am unsure to as how I need to put this into writing. Maybe state that medical expenses are being addressed and covered by my boyfriend? And add the doctors receipts from my visits?

3. I've read where some people are adding letters of descriptive situations with their applications. Is this just extra information or is there a specific section this needs to be in? I can't find any information on writing any type of letter in the guide per the forms.
Is it just add as much information as possible?

I'm not sure if I've just looked through this stuff way too much or what but I'm becoming more and more confused as to what to include, what to say and what to do in general.

Any information or help would be greatly appreciated.

1 - from the date ur visa expires.. to whenever u plan to stay
2 - I'd say no.. I believe what they mean is like HIV, Brain Damage, Bladder disease, crazy stuff.. being pregnant doesn't mean you're sick.
3 - Yes, just add information and tell them why u want to stay, how things are going to be paid, etc, etc..

Don't worry.. you're not the only one confused. Even people who has sent in their application keeps wondering if everything is ok (me lol)
