A visitor can extend their stay whilst in Canada beyond either the default 6 months usually allowed or the period less than 6 months assigned at the port of entry. For this extension from within Canada once the individual leaves the country that extension is cancelled and the individual would be assessed again on the next entry with either a 6 month stay or specific period.
An actual visa which in your example expires in 2019 would need to be applied for from scratch again in 2019 as the visa itself cannot be extended. No reason why could not apply for a new visa before the current one expires but would be a brand new application.
