Extending my work permit (PSGW - post grad work permit)

Hi everyone, I should extend my post grad work permit for another year, I did not get my full 3 years work permit as my passport was expiring soon, I have got a new passport now and want to extend the permit, couple of questions below, I appreciate your help :

1. I don't know I should apply online or in-paper? (in Canada,ca mentions in-paper) not sure though
2. IMM5710e form ,which of the options I should choose below?
I am applying for one or more of the following:
.apply for a work permit with same employer
.apply for a work permit first tome or a new employer
.Restore my status as a worker
.Get a new temporary resident permit (inadmissable applicant)
3.IMM5710e form ,what type of work permit are you applying:
Open work permit or PGWP?
4. Marital status
have files my taxes last years with status of Common-law however after I realized it is not considered as I was outside Canada for 4 months and did not fill 1 full year of living with my partner
should I mention common-law for that period of time?
