extend my stay as a visitor

hello,

now Im in canada on work permit visa... its expire on june 2015... I want to apply to stay in canada as a visitor to travel.
I have a bit problem with what I have to choose in IMM5708 for:
1. extension of my temporary resident status
2. restoration of temporary resident status as a visitor
3. initial temporary resident permit or extension of temporary resident permit


please help me
