Did you pay your consultant? You didnt submit false documents you just mistakenly filled up the application, kaya nga your asking for advice eh because there are some errors in your applications. All I can advice is call the Canadian Embassy Manila, kasi they know your case. Its not avoidable that there are some people who would tend to make mistakes in filling up the forms just like your situation and DONT worry about your past life because its never part of the CEM's business. Basta tell them everything about the form. I know its frustrating but it will all work out. Usually the embassy will deny your application if: You've commit any criminal things and if there's something wrong about your health.
