Ever been refused a visa or permit

Would US visa- 221(g) count as visa refusal even when matter that created 221(g) was overcome and visa was issued? How should one answer the statutory question in Canada EE ? Should one raise a CSE if application is already AOR-passed medical?

Just explain the situation truthfully as required. Shouldn't be an issue with an appropriate reasoning.
