I sent a web form to IRCC about completing my medicals after receiving AIP without medical request. How ever, i reside in an county where medicals is not needed (Japan).I got this reply. Not so sure about what they mean.

Lastly, if you want to stay in Canada for longer than six months, you may have to do a medical exam.
If an exam is required, you can choose a
panel physician authorized by IRCC from anywhere in the world. Please tell them where your temporary residence application will be processed so that the medical results can be sent to the right office.
The cost of the exam can be different from one doctor to another.
