sir I got same thing but it not contains my photo and also not issue and expiry date..
So is it legal or not...

Hi... I am in this exact situation. I do not have photo or issue date on my emedical report. Did you use the above mentioned report? Could you please let me know whether they accepted it. Your reply would help me to a great extend.
