Email from CIC that PR card renewal needs secondary review

Hi,
I had got RC 1 category stamp on my passport when I applied for PRTD. So on that basis I have travelled down to Canada and applied for my PR card renewal. But it has been more than 4 months and I don't have any update yet. It is still under eligibility review. So need expert opinion whether it has gone under Secondary review?

Has anyone here filled the IM 5511 form? I applied for a PR renewal and after 6.5 months the agent has asked me to fill this form and provide passport copies. The form requires me to fill the dates of my presence in Canada for the last 5 years. I have been here for around 900 days.

The problem is that the form has only 4 rows, so I am unable to capture all the dates. Can anyone guide me through this?
