Have you applied for your GCMS notes?
 
I would suggest you to order GCMS notes ASAP as there should be notes written down by the officer who was taking care of your case.
Usually there will be some explanations regarding why he/she believes your eligibility needs to be reviewed.
Once you know the elements that they have doubts about, you can start writing a letter of explanation with some supporting documents to clarify your case.
Calling IRCC agents may not be helpful as they can not share too much insight information with you or they cannot see the details of your case.

It has been more than 4 months since you applied, it is OK to order the GCMS notes. At least it should give you an idea about why it is under review, but there´s nothing unusual about that.

That's what it says on my application and has been almost 5 months. Should I worry about it?

It same on my application bit when I called them Ircc agent they told me that your eligibility is under review required and today I applied for notes

Does the officer always request additional documents during the review? Or is it always at the officer's discretion to approve/reject the application as is, without asking for clarification?

CIC agent told me today my eligibility has review required so I am obviously quite worried... have since applied for GCMS but.. any clarification you can provide would be helpful to calm nerves.

Yes. First-level assessors sometimes make mistakes that are corrected in review.

No, not always. They can either request documents if they feel there is insufficient evidence of a claim, or if they feel there are inconsistencies or credibility problems.

It depends on what the issues are with the application.

Eligibility review is very common, it just means the frontline assessor couldn´t make the call and kicked it upstairs. it does not necessarily indicate something bad about your application. There´s nothing to do until you get your GCMS notes.
