ECA evaluation, how did you deal with authorization form?

Hello,
I studied at a foreign university and they have a clear online procedure where you can request your transcripts be sent to anyone required.

The problem is both WES and ICAS seem to require that an extra Record Request Form be included in the same envelope completed by someone at the institution, this is presenting a challenge since there is no way to include extra forms in my Uni's procedures.

Did everyone here include this form? Or has anyones transcripts been rejected if they didnt have this form?

Have found out that the other ECA services make no mention of this form, they just need the trasnscripts, University of Toronto ECA service and Alberta ca ECA service, so I guess I will use one of these instead.

Even for WES, that form is optional.
