Don't use staples.

What originals are you sending that you want back? Read the instructions again. They probably require certified copies, not originals other than the work letters. No, you will not get these back.

Yes, you could print additional sheets and strike out the non-relevant fields but it's probably cleaner just to use blank paper with the question number at the top and write "Continued". You can create a similar table in a Word or Excel document that has the same headers/titles as the CIC form.
