Doubts on Filling form for skilled worker

Hi,
I request some experts to help me in completing my form for immigration application under FSW program. I am not using a representative, thats why I am having a few basic doubts.

Form- Permanant Resident Program.

1. Question No:1:- Family Name and Given Name, can some one clarify on this. In my case my name is John Mathew-- John is my name and Mathew is my fathers name. Actually I am not using Family name or Surname in my name. Later mentioned name is same in Passport too.. My family Chenatt and this is not shown as part of my name anywhere.

So what to write for Family Name:........ and Given Name:.........

2.Question No:12:- A simple doubt, do we consider our Lower Kinder Garten and Uper Kinder Garten (1 +1)= 2 years as part of education.
Or we should consider only Elementary= 10 years and Secondary = 2 years and Professional = 4 years, So total 16 years.
If we consider kinder years, it will become 18 years. Should I consider kinder years also?

3. Question No:14:- Mailing and Residential Address
Currently, I am residing in another state and as in due course, I may have to move from here due to professional reasons. So can I mention my native town/state address as mailing and residential, so in future I can avoid the need for changing of address. Please see that, I am not staying in my native state/town but I want to maintain that address, Can I do it?

4. Question No:20: Where do i intend to live in Canada?
If I mention a city after my preliminary studay and mention it here. Can I stay in some other city on getting PR Example, if I mention Ontario, Can I move to Toranto later. What is the relevance of this question.

5.Question No:21:- Family Members.
Should I mention my Parent-in-Laws too, as in future if I want to bring them to Canada. Or I can mention my spouse and my parents only in this boxes?

6.Question No:22:- If so will they accompany you to Canada.
In case of my parents, they may not accompany me to Canada immediately. They may come only after few years. So should I "Tick" on box for will they accompany me to Canada.

7. Question No:3:- Other name you have been using.
Can I mention my name which is in Church. Its something like other name.

8. Question No: 11:- Personal History till Date, what should I mention in "From - To" in case of current job. Should I leave the "To" date field BLANK or should I put date of the FORM COMPLETION DATE.

9. Question No:18: What is the relevance of address, which i lived from age of 18. Should I provide any police verification filled from all these places or is it only for information. As I am into consulting, I travel a lot and for short durations I stay in client locations for 6-8 months. If I mention all these Guest House/Hotel address, it will be a tedious job to get verifications done. Can you suggest an easy way for this. same time, I was having a house in my Base location of job. Can I put this home address for these durations together by omitting Guest House/ Hotel addresses.

10. Question No:1: Calculating Fee.
Right now, I am planning to move only my spouse along with me.
So my fee will come
principal Applicant- 550 Can Dollars
Spouse-22 years above- 550 Can Dollars

I am not planning to take my parents immediately, but in future, I may plan to move them there. Should I have to pay the fee for them now?

Hi,

i am not an expert. There may be omissions from my side. Hope you will find more guidance from some one else too:


1. There should be a family name.
So, first of all, you should go to your passport office and get your name corrected. It can be done. If you are living out of your country, go to your embassy there. Once you got the PR and in Canada adding/correcting surname is difficult process. So think about it early.

2. 16 years correct. No need to add Kinder school.

3. Permanent address is better.

4. You can choose any city in Canada except Qubec.. Better any place you have friends or relatives. You can move later according to your interest and choice.

5. The form is self explanatory. You have to mention details of your family:
Parents,spouce, children, brothers and sisters. Also similar family details of your spouce.

6. Mark only your spouce.
You have to sponsor your parents later, which will be another process.

7. Are you using that name any where in documentation?.

8. From ----," till date"

9. You should give answer to this properly which includes your education, previous job, job search period, business what ever the case may be. Make a brief record of everything. If you don't remember some don't give guess.

NO it is sixteen years

Please mention your current address is residential and permanent address as mailing address. You need to give detailes of all addresses that you sayed at after 18 years of age so no point in not mentioning your current address

Yes you can land in any city except Qubec. BTW Toronto is in Ontario state

Only your spouse and children or children of your children who are dependent on you. Your parents will be mentioned in Additional family information form and inlaws in your wife's additional family information form

These addresses are for background check. You do not need PCC from all these addresses. But you need to put in all addresses that you have rentted. I assume you can skip hotels

This will not only help you anser most of your questions but will give you right instructions to fill in the forms

From this my understanding is that, I should not write my parents details in this Tab, Am I correct?

What kind of background checks happen, as I was a bachelor, In houses, I stayed, I did not have rental agreements as s ometime I shared accomodation with friends in the same city. But, if some one call the owner, he can confirm my stay there. So In document, in which way, I should mention this?
