In the Schedule A form, there is field that asks "Family name at birth" for mother. The name in the passport is different than the family name at birth. What did you guys fill in for this?

Did you fill in the last name as in the passport or ??

Family name at birth refers to her maiden name; her passport has her current name. Fill in her maiden name in the field of the form where they ask for her family name at birth.
