Hi guys

How do we fill the Student Questionnaire Form? Does it have to be writing because i was unable to type the details..

Yes, it can be in writing also!!

but should be at least "readable"..

Abhishek

download Adobe Reader, then download the form, then open it in Adobe Reader and fill it. and plz remember to click on Validate after you fill your form .

@talha21 - I have already validated my VISA application form. I was unable to fill the Student Questionnaire Form from computer as it was not editable..

@abhi141989 - Thanks mate. I guess i'll print that form and fill it..

I also have few other questions in that Student Questionnaire Form

1) Agent / representative - there is none so N/A
2) What do i have to mention in the " Type of certificate or diploma issued(if studying) / Position title if working " Column
3) My father is financial support - So i can mention his name and the details alone right?
4) Second Language (ESL) - What do i mention here???? Because my course is Digital Animation (http://www.centennialcollege.ca/Programs/ProgramOverview.aspx?Program=6423) and it does have English language in the first two semesters...

I am very confused here.. pls help me out

yes you can fill it with Pen,

If you used to know computers and Photoshop you can do it other way (but thats tricky and time consuming, but will look better than handwritten If you have bad handwriting like mine ;)

just download student questioniarre, thats a PDF file open it with Adobe photoshop (Yes it can be opened in Photoshop), then select text tool and write anywhere You want.

If you have one day, Tommorow I can send you questioniarre form in Word Format, If you wish to.

@jay031 and 1bal - Sorry for the late reply mate.. I did fill the Student Questionnaire Form in writing. Submitted the VISA application on 29th nov.. I jus wanted to clear my head so went on a small trip and came back. My file is under processing by CHC.

good reply, but seems like not everyone knows copying in .doc format to convert into pdf and then using acrobat t fill. or converting to pdf after filling it in microsoft word. few people have virtual pdf printers and these are pretty helpful. Cutepdf is one of them.

i copy pasted the form from cic website in a word doc, filled and then printed it :D

Hello people,

Can someone let me know where I can get the student questionnaire form? Also, in the Document Checklist (on the CIC website), there is no mention of a student questionnaire form, so is this something that is optional?

Thanks!
