Doubt in Section L of OOPNP Form

Hello All,
Please help me with the following doubts while filling up the OOPNP Nominee form:

1. Section K of the form requires my sign and section L requires my spouse’s sign, right?
Then what about my kid? Do I need to put his name and sign on his behalf in section L? He is 2.5 years old, considering that section L of the form requires accompanying dependent’s statement and authorization.

2. I don’t have a middle name. Should I leave it blank or put N/A there?

3. When I look at the form after filling it, there are so many blank rows like for the dependent’s details and also section L. Do I write N/A in all the rows just to be on the safe side?
