Documents to send along with passport - TRANSIT VISA

I received a message from MyCIC to submit my passport along with passport letter request within 30 days from today. To find out where to send your passport, visit our website www .cic.gc.ca/ submit.

So after visiting this website I have two options: New York OR Los Angeles. OK

With your passport(s), make sure you include:

The passport request letter you received from CIC............OK
Any applicable VAC fees, such as passport transmission fees........WHAT IDK???
Full contact information, including a phone number and your full return address... OK
The VAC consent form (PDF, 18 KB)...OK
A pre-paid return courier envelope, if applicable...IDK AGAIN!!!

So where do I know "If applicable" condition is applicable to me or no? Since Transit visa is free of charge I am not aware what other fees to I have to pay.

On some forums I read sending to VAC is optional service, you can send directly to Visa office....OK so where is that located.
And if I have to pay any fees how do I pay it?

I am very confused.

An early and expert advise is highly appreciated and so is your past experience with this. Thank You guys

Yes VACs are optional in many countries. The Visa office is in LA or NY.

Better ask on those forums.
