Have you provided translation for Arabic text in Passport? If I remember correctly, they have mentioned to provide translations; if doc is not in English.

of course not.. it's a passport :)
all other fields are in English except the street address and the spouse name (it's up to the passport holder whether or not to include spouse name in his/her passport).

Not required for PNP, you will need one when you apply to CIC after getting ITA.
