I'm not sure if there is any specific format... or a thread where someone made a format.. I just wrote it the way I would usually write a business letter... for exp in your case:

To: Citizenship and Immigration Canada

Dear Sir/Madam,

In order to prove my relationship with my relative in Canada (my maternal uncle), please find attached the following documents:
1 - My birth certificate, showing my mother's name (mention her name here)
2 - My mother's marriage certificate, proving her father's name (mention your grandfather's name here).
3 - My uncle's birth certificate, also mentioning his father's name, therefore proving that my mother and MR. (uncle's name) are real siblings.

Please note that, due to the fact that there was no requirement to issue Birth certificate in Pakistan during the time of my mother's birth, my mother does not have this document. As a result, the only proof of father's name that is available is in the form of her marriage certificate. Therefore, I kindly request that you consider this legal document as proof of relationship.

Something like that should be enough.. and then you can sign it...
Others may be able to advise further... but this is what I would do..

You had mentioned that the score is 440... is your IELTS maximum? I would suggest to wait for next ITA, and if scores are still not going below 450 (there is 50-50 chance that it may go down), then maybe you can redo IELTS to make sure it is above 450...
Thanks alot EFK.

I have CLB 9 already.

However, I am just a bit unlucky. I have 2 years of experience. Due to this one of my adaptabilty factors is giving me 25. By next year September, I will have my CRS=465 as I can claim extra 25 for 3 yrs experience + CLB 9.

The only was I can increase score now is by doing Masters, which I cannot do as I am saving money for :)

What do you think of 440 score anyway?:)

Hmm.. makes sense..

It's not unlucky... atleast its something that will increase with time, so no worries..

I'm 50-50 about the score of 440.. realistically, based on past ITAs, 450 has been the minimum score required...
though people are saying there may be a chance of it going down to high 440s now due to change in govmnt, as liberals are less restrictive... and also that most PNPs are over for the year..... so you never know which way it can go.
So you have a total of 2 years, nothing before that? then i assume you are still young, so that's a good thing..
Meanwhile, we hope and pray that it does go down to 440s... not sure if there is anything else that can help bring the score up though in the meantime.. as the 3 years experience part is crucial for full points I guess..

Honestly speaking, I have experience of 3 years before (a total of 5 years of eperience). However, the problem is that my previous employer is not helping a bit in giving me experience letter. And I do not even have salary slips. So I cannot prove it with confidence. Even if I take chance and try to prove it, the probability is that my application is going to be rejected and I will lose ITA.

So I started a new job 2 years back. And luckily everything falls into place. Going by the documentation, My second job started after 4 months of my graduation. The new job also matches exactly with NOC title and requirements. So it is very easy for me to prove 2 years without confusing it with full-time/part-time study, as I was studying while working at my previous job. So I think everything happened in last couple of years is going to my favor. :)

I am 25 years old. I will be 26 this December. So ya, I can wait. Plus I am working so experience is increasing and even if I don't get ITA, I will keep on renewing my profile after each year. Never know when the luck strikes :).
