I called the hospital where I am going for my medical test tomorrow.

Here's the list of documents I am carrying. Can someone verify if something else is needed?

1. 5 recent passport photos with 80% face proportion.
2. Copy of my ITA.
3. Original Passport with copies of first + last page.
4. any medication that i am taking regularly.

Are there any documents one needs to carry?
 
This sounds fine. Confirm that they are on eMedical. If they aren't, cancel this appointment and got somewhere else who is on eMedical.
 
What did you tell them while booking appointment ? When I was trying to book, they told me that need UCI number or IME number. But for upfront medical exam, we dont need these forms ?

Also, is Original passport necessary ? My passport is with Indian consulate and I will be getting it back after PCC. Can Driving license act as verification of identity ?

Just tell them it is an upfront appointment under EE. Some of them might not know the program in full detail so tell them you need an appointment and you will explain in person. If they insist on a reference number, give them your EE number.

Unlikely. They would need passport copy for records and would want to see the original but check with them for alternative documentation.
 
You must call them again to confirm if original passport is required. Copies should be good enough

In my case too , the receptionist kept asking me for an IME number, which I don't have.

I am skeptical of the whole process but I am calling them again to confirm if they are on eMedical. In that case, they shouldn't need an "IME number".
