Documents Needed for Immigration Medical

Good afternoon everyone.

What documents will my spouse need to present when obtaining a medical for our Outland PR application?

We are hoping to send off her passport for renewal and may complete the medical here in Canada, while she is awaiting to receive the passport back in the mail. Would not having her passport be an issue for this?

Thank you for your help.

The medical form that you need to give the doctor is IMM 1017. It's strongly advised that you use a panel physician that uses the e-medical system, to transmit the results directly to CIC.

I'm not sure that the passport is required, if she has another form of government issued photo ID. You should contact the doctor in your area to see what is needed.

The passport IS REQUIRED. Your spouse's passport number will go right on the form that is sent to CIC. Aside from the passport you don't need anything else.

Thank you. So possibly there is a flaw in my plan.... ?

1. After the medical, I was going to send in her passport for a name change (would receive a brand new passport with Married Name)
2. Then I was going to submit the full PR outland package with her new passport

If I do the medical prior to getting a new passport and prior to submitting her package, where will the medical results go?

Likely it would be about 4 weeks between getting her medical done and then submitting her outland application... (4 weeks of waiting for passport to come back)

Please advise. Thank you

When you do your medical, you will get an e-medical receipt (a print out) which contains a number or code that will identify you. You will send a copy of this to CIC, and CIC will use that to get the medical results from the system. (The doctor's office uploads your results to the system.)

It doesn't matter if the name in the e-medical receipt does not match the name in the PR application and passport. I did my medical even before I was married, so it was in my maiden name. As long as you get the e-medical receipt and the medical was done within a year, it's fine.

Actually...it is NOT required.

From CIC form IMM1710:

Medical Examination Instructions
These medical instructions are being issued since your application has been processed to the stage
where medical examination results are now required. Please read these instructions carefully.

Please bring the following to your appointment:
- The attached Medical Report form (IMM1017E)
- Proper identification (proof of identity must include at least one government-issued document with photograph and signature, such as a passport or national identity card)


Such as, doesn't mean that it IS REQUIRED. It some countries, a driver license can be used (U.S.).

I guess that means the applicant doesn't need to provide the form, since the doctor now issues it, right?

Medical Examination Changes
CIC published OB 471 on October 31, 2012, indicating major changes to the processing of medical examination results.

Note that for spousal sponsorship, this means that the "Appendix C" is no longer being used. A new form - IMM 1017B - is used for up front medical examinations, such as those done in many spousal sponsorship cases.

There's quite a bit of interesting information contained in this OB as well, including pointers to numerous new medical forms, especially those conditions that trigger requests for additional testing and/or results from CIC.
